| By completing the enrollment form, your weekly, pay can be electronically transmitted to the bank account of your choosing.

| Paid Time Off |
|
|
As you work and accumulate hours of service you are entitled to earn credits toward paid time off. To be eligible to participate, you must first work a minimum of 500 hours. After reaching the threshold, you earn one hour of paid time for every 35 hours worked thereafter.
