Sun Home Care
provides outstanding assistance to aging seniors or other home-bound patients, enabling them to maintain their independence and dignity in a setting in which they are most at ease-their own home.
Our care coordinators get to know our senior clients and their families through a personal evaluation of their strengths and weaknesses, likes and dislikes, and long- and short-term needs. From this free, initial consultation, we create an individualized Plan of Care to support their needs. After a Plan of Care is agreed upon and implemented, our care coordinators carry out routine in-home assessments and openly communicate with family members and/or physicians throughout the term of care. |
